(1) Original and photocopy of ID card;

(2) The original Income Certificate shall be stamped with the official seal of the unit and the legal person's seal or signature;

(3) To apply for a supplementary card, the applicant for the supplementary card only needs to provide the original and a copy of the ID card;

(4) Original and photocopy of motor vehicle driving license and driving license (provided by the applicant voluntarily);

(5) Inquire about the "power of attorney" of the credit reporting (obtained by the credit reporting department);

(6) If the applicant is a private enterprise legal person, a copy of the business license, a copy of the tax registration certificate and a copy of the code certificate are also required.

(7) The original and photocopy of the medical insurance card. (Not necessary for the time being)
