1. The supplementary card applicant must be the immediate family members of the main card applicant (such as spouse, children, parents).

2. Supplementary card applicants must be at least 13 years old (inclusive).

3. Provide proof of identity (copy of ID card or household registration book).

4. The consent of the main card applicant.

5./kloc-Applicants under the age of 0/8 can only apply for supplementary cards of their legal guardians.
