1. Individual customers:
ETC stored-value card: if a stored-value card is found after the loss is reported, the individual user will know and hang it with the original stored-value card and valid identity certificate (the agent must also provide the agent's valid identity certificate), and the unit user will know and hang it with the original letter of introduction and the agent's valid identity certificate. China Bank shall handle the relevant materials for network audit and keep a copy. After the service is accepted, the Huitong Card (Stored Value Card) Service Acceptance Form will be issued, and the user will sign it for confirmation, and the Anhui Traffic Card (Stored Value Card) Service Acceptance Form (customer's copy) will be handed over to the user.
2. Corporate customers:
Corporate customers shall be handled by authorized agents with the Power of Attorney for RMB Corporate Settlement Account, the agent's ID card and the Application Form for Reporting the Loss of Business Change of Huitong Card of Anhui Branch of Bank of China.
