Just let the futures company send it to you. This thing must be filled out before it is taken to the bank. Your name, your account number and so on. If you don't know how to fill it out, you can ask a futures company or a bank.

Also, in my personal opinion, it is best not to open an account online. Find the futures companies in the province and ask them questions. You have to talk about after-sales when you buy things.
