1. Eligible applicants should provide the following information in the application form of Angel Sunshine Fund:

Materials that can prove the relationship between the applicant and his parents (or legal guardians): household registration book and ID card (copy). If the household registration book cannot confirm the guardianship relationship, it shall provide the applicant's birth certificate or the original guardianship certificate issued by the police station, the recent photo of life, and the certificate of the local civil affairs bureau or welfare home; Disabled children need the Disabled Persons' Card and relevant certificates from the local Disabled Persons' Federation.

2 rural township (town) and above people's government, city street offices and above administrative institutions issued by the family economic status certificate;

3. The report and diagnosis certificate of the child's heart ultrasonic diagnosis issued by the second-class first-class hospital;

4. Urban and rural subsistence allowances and families of martyrs need to provide subsistence allowances, copies of certificates of families of martyrs or certificates issued by local civil affairs bureaus; Orphans need to provide proof from the local civil affairs bureau or welfare home; Disabled children need the Disabled Persons' Card and relevant certificates from the local Disabled Persons' Federation.

2. To fill in the application form of Angel Sunshine Fund, please log on to the website link of China Red Cross Foundation, download and print the application form of Angel Sunshine Fund on the "Angel Sunshine Fund" page (pay attention to the latest version of the form), or log on to the website link of Tongling Red Cross Society, download and print the application form of Angel Sunshine Fund on the "Download Center" page, fill it out truthfully as required, and paste the prepared related materials at the designated location or attach it to the application form.

2. Submit the application together with the completed application form to the village committee (or neighborhood committee) of the domicile (or residence) for examination and seal, and then send it to the county or city Red Cross for examination and seal. Then the county or city Red Cross Society will report it to the provincial Red Cross Society for examination and seal, and finally the provincial Red Cross Society will submit the final opinion to the China Red Cross Angel Sunshine Fund Office.
