Serious illness medical insurance mainly includes:

1. Employee medical insurance card and serious illness medical insurance payment card;

2, serious illness medical expenses as a whole fund appropriation approval form (3) (official seal) and other materials stipulated by the serious illness medical co-ordination;

3, discharge diagnosis certificate (emergency rescue should be issued by emergency rescue diagnosis certificate), serious illness as a whole patient hospitalization medical expenses settlement list, hospitalization expenses special receipt and hospitalization expenses settlement (hospitalization reimbursement certificate);

4, special inspection, special treatment or use of expensive drugs should issue an approval form;

5. Outpatients need to issue prescriptions for correct diagnosis and serious illness and special receipts for outpatient charges in Beijing;

6, referral treatment should be provided by the hospital serious illness co-ordination office issued a clear referral;

6. The time limit for reimbursement of documents shall be based on the last day of discharge or outpatient service, within 60 days. The materials required for reimbursement of major diseases of the new rural cooperative medical system within a limited time mainly include hospitalization invoice, diagnosis certificate (stamped by the convenience office in the lobby of Building No.5), identity audit form (registered and stamped by the medical insurance office within three days of hospitalization), and receipt of application for major diseases of the new rural cooperative medical system.

Materials I need to prepare: a copy of the ID card of the participating residents, the first three pages of the entry card; List of medical expenses and diagnosis certificate (seal); Copies of discharge invoices and reimbursement vouchers of the new rural cooperative medical system (safekeeping); Copy of hospitalization medical records; To participate in my bank card or passbook, the opening bank can be ICBC, Agricultural Bank, China Construction Bank, China Bank or Postal Savings Bank.

7, a one-time reimbursement system for serious illness medical expenses, any omissions caused by enterprises and individuals, the hospital will not be reimbursed. Where the reimbursement materials are incomplete due to reasons of enterprises, individuals and hospitals, payment will be suspended.

Processing flow:

1, once hospitalized, all seriously ill patients must send the medical certificate, my basic medical insurance diagnosis and treatment manual and other materials to the medical insurance department of the hospital where they are located for registration and verification, so as not to affect the reimbursement of hospitalization medical expenses;

2. To apply for outpatient reimbursement of 23 diseases such as liver cirrhosis, the insured residents should fill in the relevant forms at the designated hospital's medical insurance department for preliminary examination with their basic medical insurance diagnosis and treatment manual and the materials required for reporting diseases (see local policies for details) within the specified time;

3. The designated hospitals will report the information of the insured residents who have passed the preliminary examination to the urban medical insurance agency for review.
