application procedures

1. Applicants need to download the application form for assistance from Yan Ran Angel Fund through the official website of China Red Cross Foundation, and fill in the form (including the following contents) on the premise of fully understanding the Application Instructions for Yan Ran Angel Fund: (1) A copy of the patient's identification document; (2) in the case of children, it is also necessary to provide the relationship certificate of the legal guardian of the child; (3) An application form for funding from Yan Ran Angel Fund filled out by the legal guardian of the patient or child; (4) Preliminary physical examination report of patients in medical institutions at or above the county level; (5) A child with cleft lip and palate from a poor family under 2 years old who is not from agricultural registered permanent residence must have a certificate of family economic status issued by an administrative agency above the town street office. 2. The application materials shall be submitted to the project management office of Yan Ran Angel Fund of China Red Cross Foundation.
