1. After receiving and filling in the form for enrollment registration, the manager of the insured unit will copy the Application Form for Municipal Social Security (Citizen) Card on both sides and send it to each new insured, and instruct the new insured to fill out the application form according to the requirements of the application rules. 2. Before the 3rd day of the month following the enrollment registration, the manager of the insured unit will arrange for the new insured person to take his ID card (or military officer's card and passport) to the designated photo studio of the social security (citizen) card to take digital photos of the social security (citizen) card, and paste the photo receipt in the corresponding position on the application form. The fee for taking photos of social security (citizen) cards is 4 yuan/person. III. Collect the application form and the cost of work. Please ask the manager of the insured unit to collect the completed XX Social Security (Citizen) Card Application Form from the new insured before 19th of the month following the registration, and collect the cost of social security (citizen) card for 2 yuan/person at the same time.

iv. Submit the form, pay the fee and get the card. The bank where the medical insurance account is opened will inform the insured unit to go to the medical insurance center to handle the formalities of submitting the form, paying the fee and getting the card after the 19th day of the next month of enrollment. The process of submitting forms, paying fees and receiving cards can be concentrated in the designated windows of the service hall of the medical insurance center.

5. The agent of the card-issuing insured unit shall issue the social security agent within one week after receiving the card, and the social security agent shall issue the social security (citizen) card to the insured within one week after receiving the card.

VI. Check the name, resident ID number and other information on the card and passbook after receiving the card. If the information is incorrect, please change it at the social security fund center where the insurance formalities are handled as soon as possible. In addition, be sure to change the password.
