(1) Copy of patient identification document; (2) For children, the relationship certificate of the legal guardian of the child is also required; (3) The Application Form for the Grant of Ranyan Tianshi Fund filled out by the legal guardian of the patient or child; (4) Preliminary physical examination report of patients in medical institutions at or above the county level; (5) Children with cleft lip and palate from poor families in agricultural registered permanent residence under the age of 2 must have a certificate of family economic status issued by the administrative organ at or above the town street office.
