To apply for maternity insurance, the company needs to issue the following certificates:

The application materials that female employees need to submit are:

2. Birth (death) certificate issued by the medical department (original and photocopy);

3. My ID card (original and photocopy) of female workers and family planning operators;

4. Approval Form for Maternity Medical Certificate of Enterprise Employees;

5, enterprise employees family planning surgery medical certificate application form;

6, enterprise employees maternity medical expenses reimbursement application form;

7, maternity insurance benefits of enterprise employees approved settlement table;

8, enterprise employee maternity insurance on-site medical application form;

9, maternity medical bills, cost list, outpatient medical records, discharge summary and other raw materials;

10, receipt.

Article 54

The employer has paid maternity insurance, and its employees enjoy maternity insurance benefits; Unemployed spouses of employees enjoy maternity medical expenses in accordance with state regulations. The required funds are paid from the maternity insurance fund.

Maternity insurance benefits include maternity medical expenses and maternity allowance.

Article 56

Workers in any of the following circumstances can enjoy maternity allowance in accordance with state regulations:

(1) Female employees enjoy maternity leave;

(2) Enjoy family planning operation leave;

(3) Other circumstances stipulated by laws and regulations.

Maternity allowance is calculated and paid according to the average monthly salary of employees in the previous year of the employer.
