The depositor of the provident fund shall apply to the service hall of the housing provident fund center at the place of deposit with his ID card. Staff will check the information and report the loss. After 3 working days, you can get a new housing provident fund card (the account number remains unchanged) at the service hall of the housing provident fund center at the place of deposit with your ID card and the signed Application Form for Reporting the Loss of Housing Provident Fund Card.
If the depositor applies to report the loss orally or by mail first, he must go through the formal reporting procedures within 5 working days after the housing provident fund center accepts it, and the overdue system will automatically restore the account from the reporting state to the normal state.
To apply for a provident fund card, the following conditions shall be met:
1, state organs and institutions;
2. State-owned enterprises, collective enterprises, enterprises with foreign investment, enterprises with investment from Hong Kong, Macao and Taiwan, private enterprises and other enterprises and economic organizations;
3. Private non-enterprise units, foundations, social organizations, law firms, etc. ;
4, foreign and Hong Kong, Macao and Taiwan enterprises and other economic organizations resident representative offices in this city.
