In order to alleviate the medical difficulties of major diseases of children from poor families in our city, we should give full play to the supplementary role of charitable assistance in the social security system, carry out charitable medical assistance that is connected and complementary with the basic medical insurance system for urban residents, the new rural cooperative medical system and the urban and rural medical assistance system, and gradually form a serious illness assistance mechanism for poor children that is compatible with the social and economic development level of our city.

Second, the implementation principle

1. In principle, the raising of charitable relief funds for children with serious illness in poor families is mainly based on government investment, supplemented by social donations;

2. To help children suffering from serious diseases in poor families, adhere to the principles of basic medical insurance system, urban and rural medical assistance system as the foundation, charitable assistance as the supplement, and the principle of fixed income and expenditure, balance of payments, and acting according to one's ability;

3 to determine the target of assistance, adhere to the principle of enjoying the minimum living guarantee and helping children from poor families.

Three. Sources and management of funds

The charitable relief fund for serious illness of children from poor families in the city comes from the special financial funds of the city, the welfare lottery public welfare fund and the funds raised from the society. The total fund is not less than 6,543,800 yuan. The account is located in the city charity association.

District (county) poor families children's serious illness charity relief fund, by the district (county) financial special funds and social funds raised. The account is located in the district (county) charity.

City, district (county) poor families children's serious illness charity relief fund, the implementation of account management, earmarking, and shall not be used for other purposes. The collection and use of funds should be announced regularly, and consciously accept the supervision of donors and discipline inspection, auditing and financial departments.

Five, the relief object

1, must be a child with the following conditions.

(1) has the household registration in this city and has participated in the basic medical insurance for urban and rural residents in this city, and is under the age of 16 (inclusive);

(2) families are poor families who enjoy urban and rural subsistence allowances or special assistance from civil affairs departments;

(three) suffering from major diseases within the scope of rescue diseases, hospitalization in designated hospitals (including special diseases in designated hospitals);

(4) According to the "Implementation Opinions on Further Improving the Urban and Rural Medical Assistance System", after giving assistance, the personal burden is still heavy.

Sixth, the amount of aid.

Children's medical expenses incurred during hospitalization in designated hospitals (or treated as entitled groups) that meet the basic medical insurance drug list, diagnosis and treatment project list and medical service facilities list in the current year (referring to the year of applying for assistance, not the whole year) shall be paid by the basic medical insurance, reimbursement of large medical expenses and the implementation opinions of urban and rural "medical assistance system", and the part of their personal burden accumulated over 2,000 yuan shall be paid by the charity relief fund for serious illness of poor children in cities, districts and counties respectively.

City, district and county poor children's serious illness charity relief fund, the total annual relief does not exceed 50 thousand yuan.

Seven. Application and approval of assistance

To apply for assistance, the guardian of the child shall submit a written application to the county charity association and truthfully provide the following supporting materials (original):

(1) children's household registration book and age certificate (household registration book);

(2) proof that the children have participated in urban and rural medical insurance or the "new rural cooperative medical system";

(3) The diagnosis certificate issued by the designated hospital and the corresponding case data;

(4) Proof that the children's families enjoy the minimum living guarantee or poor relief (Tianjin minimum living guarantee card, proof that children enjoy the minimum living guarantee or Tianjin urban and rural residents' poor relief card, proof that children enjoy poor relief);

(5) Special receipt for medical insurance for personal treatment expenses in the current year (excluding documents of the year of non-approved assistance);

(6) In the same period, according to the "Implementation Opinions on Further Improving the Urban and Rural Medical Assistance System", the special receipt for medical insurance for the amount of assistance was accepted;

(seven) other relevant supporting materials that need to be supplemented.

The applicant's written application and supporting materials shall be examined by the district (county) charity association, and the applicant shall fill in the Application Form for Charity Relief for Serious and Serious Diseases of Poor Children.

The district (county) charity association shall complete the investigation and verification within 15 working days after receiving the application. To meet the conditions of assistance, the district (county) children's serious illness charity relief fund shall provide assistance according to the prescribed proportion, and all application materials (including the "Application Approval Form" signed by the district (county) relief fund) shall be reported to the municipal charity.

After receiving all the information, the Municipal Charity Association will fill in the Application Approval Form for Serious Illness Relief for Poor Children in Tianjin, complete the investigation and verification within 10 working days, put forward the funding opinions, and distribute the funding to the children or their guardians.

Eight. others

The relief from the charity relief fund for serious illness of poor children can be divided into several times and accumulated to 50 thousand yuan. The interval between each declaration and the payment of relief funds is not less than 2 months.
