4. Application procedures 1. After receiving this notice, the applying unit should bring a USB flash drive to our bureau to copy the payment base table (electronic document) of insured units in December 2013.

2. The reporting unit should fill in the "Details of Salaries of All Employees in 2013" and the "Declaration Form of the Payment Base of Social Insurance Insured Employees in 2014" for the insured employees of the unit in the format specified above.

3. After reviewing the data and information reported by the reporting unit, our bureau will promptly import the data that meets the requirements into the system and print the "Social Insurance Employee Contribution Base Validation Form" (in duplicate), which will be submitted to the handling personnel of the reporting unit

5. Determination of payment base After the declaration work is completed, our bureau will determine the payment base of each insured unit based on the declaration situation, and implement the new payment base in January 2014.

After the statistics bureau of the autonomous region announces the average salary of urban employees in the autonomous region in 2013, our bureau will re-determine the maximum and minimum payment bases based on the average salary of employees in the autonomous region in 2013.

6. Relevant requirements 1. Each unit must truthfully declare the payment base and be responsible for the authenticity and legality of the relevant report materials submitted.

The declared personal payment base of insured employees must be disclosed in accordance with regulations.

2. The written information reported by each unit should be uniformly on A4 paper and stamped with the official seal of the unit.

The "2014 Social Security Insurance Employee Payment Base Declaration Form" should be submitted in the form of written and electronic documents. The name of the employee must be filled in according to the name on the ID card, and the ID card number must be filled in with 18 digits. Before submission, it must be approved by this

Confirmation and announcement of unit employees.

3. For insured units that are in arrears with social insurance premiums, they should first make up the arrears according to regulations, and then make a base declaration.

4. Non-retired and retired part-time and part-time employees shall declare and pay work-related injury insurance by their part-time units.

5. Our bureau will investigate and deal with any employer that provides false information during the declaration process, deliberately conceals the total salary and number of employees, or refuses to provide information for various reasons.
