The goal of Smart and Connected Health (SCH) program is to accelerate the development and use of innovative methods and support the transformation of medical care from reactive and hospital-centered to preventive, forward-looking, evidence-based, people-oriented and concerned with the urgent needs of well-being rather than disease. The method of combining technology-based solutions with biological behavior health research has been supported by many federal agencies, including the National Science Foundation (NSF) and the National Institutes of Health (NIH). The purpose of this plan is to develop the next generation of health care solutions and encourage existing and new research groups to focus on breaking through ideas in various fields that are valuable to health, such as sensor technology, network, information and machine learning technology, decision support systems, behavioral and cognitive process modeling, and system and process modeling. Effective solutions must meet a large number of constraints caused by clinical/medical needs, social interaction, cognitive limitations, behavioral change obstacles, data heterogeneity, semantic mismatch and the limitations of the current network physical system. This solution requires multidisciplinary teams to be ready to solve technical, behavioral and clinical problems from basic science to clinical practice.
