The materials submitted include: the application approval form for epilepsy fund of Guizhou Poverty Alleviation Foundation, the original and photocopy of the applicant's or patient's guardian's ID card, the patient's family household registration book and its photocopy, the patient's diagnosis certificate in Guiyang Cerebral Epilepsy Hospital, the patient's family difficulties certificate and medical bills.

3, rural township (town) above the people's government or town street offices above the administrative institutions issued by the family economic status certificate;

4. Inspection reports, diagnosis certificates and medical bills issued by patients;

5. Guizhou Poverty Alleviation Foundation will review the applicant's application qualifications and related materials, and if it meets the poverty alleviation fund distribution standards, it will directly allocate relevant funds to the applicant.
