⑹ Overseas medical insurance policy (can be purchased from domestic insurance companies).

⑺ Provide 1 original copy of the "Re-employment Certification Letter from the Unit".

If you are a student who has not yet worked, please provide an original copy of the certificate from the institution you are attending.

⑻Notarized certificate ①Notarized birth certificate.

② Notarization of no crime.

③ Highest academic qualification (degree) notarized.

④ Notarization of work experience: from the date you started working to the present; if you are a student, please provide proof of the school you attended.

⑤Notarization of name change (if the name has been changed before).

Note: The above notarizations must be in Chinese and English.

Students studying abroad can apply at the first-level notary office of the city where they are located.
