1, product name. Including the Chinese and foreign names of imported products. The Chinese name of a product consists of three parts: a trademark name, a generic name and an attribute name, which shall comply with the provisions of the Measures for the Administration of Cosmetic Labeling.

2. Product category. The registration applicant and the applicant shall determine the product classification code and product declaration category according to the Regulations on the Supervision and Administration of Cosmetics, the Standards for the Classification of Cosmetics and other relevant laws and regulations. Where there are claims of multiple functions, those with special functions shall be declared as special cosmetics.

3. Production enterprise information. If the domestic registration applicant and the filing person are self-produced, the name, address and production address of the enterprise shall be filled in with the name, domicile and production address information specified in the cosmetics production license.

4. (Confirmation of Entrusted Production Relationship) When applying for cosmetics registration or filing, if there is entrusted production, the entrusted production relationship shall be confirmed and the entrusted production agreement shall be submitted.
