an application for changing the name and address of the trademark applicant, the contact address of the registrant, the collective trademark certificate and the trademark management rules, or an application for changing the recipient of the trademark agent's documents, and a copy of the resident identity certificate, legal person qualification certificate or other organization's identity documents (such as a copy of the business license of the enterprise, a natural person's ID card/a residence permit for Hong Kong and Macao residents/a residence permit/passport for Taiwan Province residents, etc.). If the name of the registrant is changed, the certificate of change issued by the registration authority shall be submitted.

if it is entrusted to a trademark agency, a power of attorney for trademark agency shall be submitted. If the application document is in a foreign language, it shall be accompanied by a Chinese translation, which shall be confirmed by the signature of the applicant, agency or translation agency.
