1. "Application Form for Change of Registration for Licensed Pharmacists" in duplicate; in the column of assessment opinions of the practicing unit, truthfully fill in the applicant's ethics , ability, diligence and performance, signed by the person in charge of the unit and stamped with the official seal of the unit to be valid; log in to the State Food and Drug Administration’s Licensed Pharmacist Registration Platform to fill in, print and submit;

2. The original copy of the "Practicing Pharmacist Qualification Certificate" and Copy;

3. Original copy of "Registration Certificate for Practicing Pharmacist";

4. Copy of proof of legal opening of the new practice unit (drug production and operation license or medical institution practice license) );

5. Three recent one-inch, full-face, half-length color photos of the same page without a hat;
