Fill in the instructions:
1. This book is applicable to the application for correction of trademark application/registration matters. The application should be typed or printed. The applicant shall fill in the form in accordance with the regulations and may not modify the format.
2. The name of the applicant and the seal (signature) affixed to the seal (signature) of the applicant shall be consistent with the name in the identity document. If the applicant is a natural person, the certificate number should be filled in after the name.
3. The address of the applicant should be preceded by the name of the administrative division such as province, city and county. The applicant should fill in the address on the identity document. If the address in the identity document is not marked with administrative divisions such as province, city and county, the applicant shall add the name of the corresponding administrative division. If the applicant is a natural person, you can fill in the mailing address.
4. Where a trademark agency is entrusted to declare, the name of the agency shall be filled in the "agency seal/agent signature" and signed by the agent, with the agency seal affixed.
5. The trademark application number (registration number) shall be filled in when applying for correction.
6. If the name/address of the trademark applicant is corrected, all categories of trademarks shall be filled in.
7. The corrected contents do not involve the substantive contents of the trademark application documents or registration documents. The items to be corrected shall indicate the items and contents of the application for correction.
8. If you need to reissue the certification documents, you should choose "Yes" and attach the wrong trademark registration certificate or the original documents such as the certificate of change, transfer and renewal.
9.*** Where there is a trademark application to correct the trademark/registered items, the representative shall apply.
10. If the applicant is a legal person or other organization, it shall affix a seal at the "seal (signature) of the applicant". If the applicant is a natural person, he should sign here. The seal or signature shall be complete and clear.
