III. Contents of declaration and registration
The Declaration Form for Discharge of Pollutants (for Trial Implementation) is divided into two parts: basic information declaration and dynamic information declaration. The declaration of basic information generally includes the enterprise legal person, address, contact telephone number, main product name, etc. Dynamic information declaration mainly refers to the information and data related to the verification of sewage charges, such as product output and pollutant discharge in a production stage of an enterprise.

VI. Precautions
1. Cancel the annual pre-declaration. When a pollutant discharge unit declares for the first time or the basic situation changes, it shall submit the relevant Declaration Form for Basic Information of Pollutants Discharge (Trial) to the environmental monitoring agency responsible for the collection and management of sewage charges, fill in the relevant Dynamic Declaration Form for Pollutants Discharge (Trial) according to the actual monthly or quarterly pollutant discharge, and report to the environmental monitoring agency responsible for the collection and management of sewage charges within 7 working days after the end of each month or quarter, and provide relevant information on pollutant discharge.
2. Implement classified declaration. The declaration types of pollutant discharge units are divided into: (1) general pollutant discharge units; (2) Centralized sewage treatment unit; (3) Specialized solid waste disposal units; (4) Iron and steel enterprises; (5) the construction unit; (6) Small tertiary industry; (7) Livestock and poultry farms.
(Note: the construction unit can register enterprise information with "construction permit number" instead of "organization code". )
3. Implement online declaration. Pollutant discharge units declare pollutants through the network, and the data are synchronized to the information management system of the whole process of sewage charge collection, which is audited and determined by the competent environmental monitoring agency. Construction units, small tertiary industries and other sewage units that are difficult to declare online can fill in paper forms, and the environmental monitoring agency responsible for the collection and management of sewage charges will assist in online declaration.
VII. Materials to be provided for declaration
1. The following materials shall be provided at the same time when the basic information is declared:
(1) Declaration Form for Basic Information of Pollutant Discharge (for Trial Implementation) (in duplicate);
(2) A copy of the business license and organization code certificate of the enterprise legal person or individual industrial and commercial household;
(3) EIA report and approval document, environmental protection completion acceptance report and approval document;
(4) pollutant discharge permit;
(5) Drainage permit;
(6) hazardous waste management (transfer) plan filing form and hazardous waste entrusted disposal agreement;
(7) Design scheme of pollution control facilities.
2. When applying for dynamic information, the following materials shall be provided at the same time:
(1) Application Form for Dynamic Discharge of Pollutants (for Trial Implementation) (in duplicate);
(2) dynamic reporting period product output, raw and auxiliary materials consumption certificate;
(3) photocopies of water bills and electricity bills during the dynamic reporting period;
(4) photocopies of fuel use documents and fuel inspection reports during the dynamic reporting period;
(5) a copy of the statistical table of online monitoring data of pollution sources or the supervision and monitoring report during the dynamic reporting period;
(6) The entrusted hazardous waste disposal unit shall provide information such as the hazardous waste transfer list in the dynamic reporting period.
(7) The data to be measured by the pollutant discharge unit in the declaration form shall provide the calculation description of the data.
