When directly applying for the registration of a certification trademark, in addition to submitting the Application for Trademark Registration filled out and printed in accordance with the regulations and stamped with the applicant's official seal, trademark pattern, copy of identification document (confirmed by the applicant's seal) and copy of the ID card of the handler, the management rules for the use of the certification trademark should also be submitted, and the professional technicians and professional testing equipment owned by it or the agency entrusted by it should be explained in detail, so as to show that it has the ability to supervise the quality of the specific goods certified by the certification trademark.
Documents to be submitted:
1. For company applications, please fill in the name and address according to the business license and affix the official seal; for individual applications, please fill in the name and address according to the ID card and sign it;
2. Trademark pattern: If no color is required, please submit 6-1 black and white patterns; If the color is specified, please submit 6 copies of color pattern and 1 copy of black and white pattern. The trademark pattern should meet the requirements of length x width not more than 1X25px and not less than 5X125px；.
3. Copy of license (company application)/copy of personal ID card, and copy of business license (individual application);
4. supporting documents: the trademark registration of cigarettes, cigars, packaged shredded tobacco, medicines for human use, medical nutritious food, medical nutritious drinks, baby food and other commodities shall be accompanied by relevant supporting documents.
5. companies and enterprises can only apply in the name of the company, and individual applications need to submit a copy of the business license of individual businesses.
