Scope of acceptance

Enterprises that have obtained the "Health Food Approval Certificate" from the Ministry of Health and produce health food locally.

Acceptance conditions

1. The product has obtained the "Health Food Approval Certificate" from the Ministry of Health.

2. Production and processing enterprises shall comply with the provisions of "Good Manufacturing Practice for Health Food" (GB 17405-1998).

3. The application materials submitted are complete, legal and valid.

Application materials (in triplicate):

1. Health Food Hygiene License Application Form;

2. "Health Food Approval Certificate" from the Ministry of Health;

3. Product quality standards (enterprise standards);

4. Production enterprise hygiene standards and formulation instructions;

5. Product formula and basis;

6. Production process and diagram;

7. For entrusted processing, a valid entrusted processing contract signed between the holder of the "Health Food Approval Certificate" and the entrusted processing enterprise shall be submitted;

8. Introduction to production conditions, production technicians, and quality assurance systems;

9. Samples of product design packaging and product instructions;

10. Product hygiene monitoring data.

11. Other information, business license, local health license, trademark registration certificate, etc.

Notes:

1. The product name appearing in the application materials should include the product brand and model.

2. In addition to the application form and inspection report, all application materials should be stamped page by page with the official seal of the reporting unit (it can be a seam seal)

3. The application form must be written in pen Or fill in with a carbon pen, fill in the blank with the word "none"; other application materials should use A4. Print on standard paper (it is recommended to use small 4-point Song fonts for Chinese and 12-point fonts for English). All declared contents should be complete and clear and must not be altered.

4. The copies of application materials should be copied from the originals, and their clarity should be exactly the same as the originals.

5. The hygienic monitoring data of the products are provided by the inspection agency determined by the Autonomous Region Health Department. Supervisors sent by the District Health Supervision Office will take samples of three production batches on site, and the applicant will submit them for inspection.

6. If you use nationally protected wild animals and plants as raw materials, you must provide proof of the legal use of wild animals and plants.

Processing time limit:

1. A decision on whether to accept the application will be made within 5 working days after receiving the application materials;

2. The date of acceptance if the application materials are passed. Submit to the Department of Health for approval within 30 working days.
