3. The explanation of the product difference of each model of the same applicant is "provided by the main engine factory";

4. Copy of the original 3C product certification certificate provided by 4.OEM (valid 3C certificate is required);

5. The original "China Compulsory Product Certification Factory Review Report" for 3C product certification provided by 5.OEM (qualified and valid);

6. Copy of 3C certified type test report of original 3C products "provided by OEM";

7.3C certificate applicant's business license and organization code certificate shall be provided by the applicant (3C certificate applicant, manufacturer and manufacturer are not the same enterprise, so the business license and organization code certificate of manufacturer and manufacturer shall be provided separately);

8. ODM OEM 3C Certification Intention Application Form and 3C Certification Formal Application Form (after the applicant submits the ODM OEM 3C Certification Intention Application Form, the certification institution will provide the original, and the enterprise only needs to sign and seal it) and "provided by the applicant";

9. Copy of the 3C certification certificate of the parts in the 3C certification catalogue provided by OEM;

10, the original ODM OEM agreement "provided by the applicant" (sample OEM OEM agreement for 3C certified products);
