You just need to go to the local notary office, pay a certain fee, and then the notary office will issue you a statement stating that you want to use your personal portrait as a trademark pattern. Then when you apply for a trademark, you can attach this notarized statement to the application documents. The notarized statement shall be provided in two originals.
