(5) Corporate credit certificate issued by the bank where the account is opened. It is issued by the enterprise after checking the credit reporting system of the People's Bank of China with the account-opening bank, and specifies whether the enterprise has any problems with bank credit in 2012-2013.
(6) Enterprise tax status certificate issued by the national taxation department and local taxation department at or above the county level (inclusive). Explain whether the company had any tax-related illegal issues from January 2012 to December 2013.
(7) Quality and safety certificate issued by the agricultural or other statutory regulatory authorities at or above the county level. Explain whether the company had product quality and safety problems in 2012-2013.
(8) The situation of farmers driven by enterprises issued by agricultural economic stations at or above the county level. Explain the ways in which enterprises drive the interests of farmers, the number of farmers they drive, and the number of farmers they drive to increase their income from 2012 to 2013.
If an enterprise has the following circumstances: obtains the "Three Products and One Standard" certification, obtains export registration, has a specialized R&D institution, obtains scientific and technological awards or honors at or above the provincial level, passes ISO9000, HACCP and other quality certifications, and obtains China Well-known trademark or Chinese famous brand product certificate, provincial famous brand product or famous trademark brand certification, etc., relevant explanations should be provided and a copy of the certificate should be attached.
If the monitoring enterprise has changed its name, it is also necessary to provide materials indicating that the industrial and commercial administration department agrees with the enterprise's name change, a copy of the enterprise's business license after the name change, and an explanation of whether the enterprise's main business and assets have changed. The company name and official seal used when reporting monitoring materials should be consistent with the business license of the company after the name change.
If the paper materials provided by the company are copies, scans, color prints, etc., the company's official seal should be stamped on each page of the copy to ensure that the reported copy is consistent with the original.
(2) According to the declaration and monitoring status, summarize the "Operation Status Table of Provincial Key Leading Enterprises in Agricultural Industrialization Operation" and submit it to the Provincial Industry Office with an official seal, and submit it to the monitoring of provincial key leading enterprises at the same time Analysis report, the main contents include: monitoring results (the relevant operations and monitoring results of the yellow card warning enterprise during the warning period must be specifically explained), the operation and development of the enterprise, and the achievements achieved (especially the promotion of agricultural efficiency and driving farmers (increasing income, improving science and technology and the quality and safety level of agricultural products, etc.), its role and existing problems, the reasons for the emergence of unqualified enterprises, and suggestions for the next step.
