1. Original identification materials of the declarant:
① Natural persons provide identity documents and household registration books, and applicants with collective household registration provide the original page of the Permanent Population Registration Card and a copy of the first page stamped by the household registration unit;
② the legal person shall submit the legal person qualification certificate (i.e. business license and articles of association), the legal representative's identity certificate and the legal representative's identity card;
2. Copy of the transferee's identity documents:
① A copy of the identity card provided by the natural person;
② the legal person shall submit a copy of the business license.
3. Rights certificates such as trademark registration certificate;
4. If a natural person transfers a trademark, it is also required to provide proof of marital status:
① Married, provide a marriage certificate, and both husband and wife must apply for notarization together;
② remarriage after divorce, and provide the divorce certificate, divorce agreement or the court's judgment, conciliation statement and legal documents of the Civil Affairs Bureau;
③ If the widow does not remarry, provide the original marriage certificate and spouse's death certificate;
5. If the legal person transfers the trademark, it shall also submit the resolution of the meeting of the competent decision-making body within the company (the shareholders' meeting or the board of directors according to the articles of association) agreeing to transfer the trademark;
6. the text of the declaration (the notary office can provide the substitute book);
7. If the evidential materials provided by you are insufficient to prove the notarized matters, please supplement the evidential materials according to the requirements of the notary.
