3. An enterprise applying for a credit card shall fill in a credit card application form with the issuing bank.
4. When filling in the application form, the cardholder's name, age, address, enterprise property and liabilities, occupation, position, etc. shall be specified in detail and sent to the issuing bank for review.
The issuing bank will carefully examine the application items and contents received. Upon examination, if it meets the conditions for issuing cards, it will be approved to issue cards, and a credit card notice will be issued to the applicant, informing the applicant to go through the account opening formalities at the issuing bank.
6. When opening an account, the applicant should deposit a credit card deposit first, then pay the annual fee, and sometimes deposit a certain amount of money.
7. If the applicant deposits in a credit card and settles by transfer, he shall fill out the bill in duplicate; If the deposit is made in cash, fill in the cash payment form in duplicate and fill in the deposit amount.
8. The annual credit card fee ranges from 10 yuan to 100 yuan, and the regulations of the issuing banks vary. When paying the annual fee, you can fill in the triplicate settlement service fee certificate and fill in the amount of the annual fee paid.

After the applicant fills in the deposit slip or cash payment slip (for depositing credit card deposits and deposits) and the payment voucher for settlement business according to the regulations, he will hand over the transfer check or cash payable together with the applicant's ID card and credit card notice to the credit card department of the issuing bank according to the total amount of credit card deposits, annual fees and deposits.

12. After receiving the credit card, the cardholder shall immediately sign the back of the card according to his customary signature style, and shall not alter it after signing.
