Key entitled groups and urban and rural residents shall provide valid certificates and photocopies issued by the civil affairs department; Disabled persons with difficulties should provide valid certificates and copies issued by the Disabled Persons' Federation; Families with special difficulties need to provide a certificate issued by the domicile (the city's household registration) or the village or community where Dongguan lives (not the city's household registration) and audited by the civil affairs department of Jie Zhen.
4. The original and photocopy of the medical expense document (hospitalization certificate or invoice) or social security settlement document issued by the hospital.
5. Provide the original and photocopy of the bank card or passbook that matches the name of the applicant's ID card. (Except the medical expense documents or social security settlement documents issued by the hospital, the original documents are only for inspection. If it is consistent with the original, the receiving unit shall affix the seal of "consistent with the original" on the corresponding copy, and the original shall be returned to the applicant after inspection. )
Second, apply for charity.
The applicant applies to the Municipal Charity Association with relevant application materials, and the Municipal Charity Association is responsible for reviewing and confirming the application materials, and publicizing the application materials that meet the rescue conditions for 3 days. If the investigation is true and there is no objection, it shall be submitted to the evaluation team of "City Welfare Lottery Care Fund" for approval.
