The national poverty alleviation milk powder needs the guardian's autograph and the original official seal after being approved by the village and township levels to fill in the "Approval Form for Subsidies for Poor Infant Milk Powder in the National Milk Poverty Alleviation Project"; Copy of birth medical certificate or identity card (household register) of the sponsored baby (1 option 3), copy of guardian's identity card and official seal of the people's government at or above the township level; Poverty certificate issued by the people's government at or above the township level (except for households who have not been lifted out of poverty); A copy of the ID card of the child welfare director of the village where he is located and stamped with the official seal of the Township People's Government; Photo of the child welfare director and the sponsored baby and guardian in the village with ID card (or household registration book). (The photo is uploaded to the website of the National Dairy Poverty Alleviation Project Management Platform, and a paper version is not required. )

In order to speed up the implementation of the national milk source poverty alleviation project and the child welfare director project, and implement the policy welfare for the vulnerable groups, the village, residential and community welfare directors are requested to submit the confirmed information of infants aged 0-3 to the Foundation for examination and approval in time according to the following operating procedures, so as to facilitate the subsequent distribution of milk powder and other matters.

The whole process must be completed by the welfare directors (women's Federation presidents) of villages, residences and communities. For families in need, please consult the welfare director in the village, and the head of the household only needs to declare through the welfare director.
