Attachment: In order to ensure that users can apply for the "China Electronic Port" IC card in a timely manner, please apply for the unit to prepare the following materials before applying for the card:

1. Fill out the "China Electronic Port" IC card as required Enterprise Information Registration Form" (Form No. 1) and "China Electronic Port Enterprise IC Card Registration Form" (Form No. 2). Form No. 2 can be copied and filled in by yourself as needed.

2. The electronic port data center identification card must be presented when applying. Users who have not applied for an identification card can go to Pudong Customs East 211 to pay to get the registration form.

3. The following supporting documents are required when applying:

1. Copy and original of "Organization Code Certificate of the People's Republic of China" (must pass annual review)

2. A copy of the "Enterprise Legal Person Business License" with the official seal and original copy of the unit (actual funds must not be 0)

3. Copies and originals of "Tax Registration Certificate" or "Tax Registration Certificate of Foreign Speculative Enterprises" (national tax)

4. Copies and originals of the "Qualification Certificate of the People's Republic of China for Import and Export Enterprises" or the "Approval Certificate of the People's Republic of China and Foreign-Invested Enterprises" (the actual funds received must not be 0)

5. Copies and originals of the "Customs Declaration Unit Enterprise Registration Certificate" (if the customs inspection has expired, it can only be processed after the customs inspection)

6. Original foreign exchange underwriter certificate or “registration invoice”

7. Original copy of customs declaration certificate (customs declaration authority is enabled)

8. Original directory card (enabled import foreign exchange payment authority)

9. Original copy of the foreign exchange verification form receipt certificate (authorization for export collection of foreign exchange is enabled)

Note:

All copies must be stamped with the official seal.

All the above certification materials must pass the annual review by relevant departments.

The contents of Form No. 1 and Form No. 2 must be filled in in detail. For the information of corporate managers in Form No. 1, fill in the legal person or managerial personnel of the unit.

Taiwan compatriots’ ID cards cannot be used. To fill in the Taiwan compatriot ID number, a copy of the Taiwan compatriot ID card is required.

The actual registered capital when applying for a new enterprise shall not be 0.

Enterprises whose registered address is in the Waigaoqiao Free Trade Zone do not need to provide materials 6, 8, and 9 when applying.

IV. To apply for an electronic port IC card, you need to bring the following funds:

1. Electronic port IC card: 170 yuan/card (at least two per unit)

2. Card reader: 280 yuan/each

3. Entry fee: 30 yuan/copy

4. Electronic port installation software: 300 yuan/set

5. Telecom Internet access fee: (choose one)

(1) Dial-up time card: 200 yuan/card, 500 yuan/card (0.3 yuan/minute)

(2) Annual card: 2,400 yuan/card (1,200 hours/year)

(3) Broadband Internet card: 720 yuan/card (half a year)

5. You can Bring the above information and funds to Room 101, Pudong Customs East, No. 153, Lujiazui West Road (next to the Oriental Pearl Tower) to go through the card entry and production procedures.
