If the trademark applied for change is a trademark of * * *, the following documents shall also be submitted: (1) If there is a trademark representative in the application for change of * * *, an application for change of the name/address of the registrant shall be filled in, and a statement of consent of other * * * owners shall also be submitted. (2) If an application is made to change the name of the trademark * * * *, whether it is to change the name of all * * * owners or part of * * * owners, or to change the name of * * * owners, the names of all * * * owners shall be filled in the application form.
