1. Fill in the "Applicant Name" column: Enterprise: Fill in the enterprise name, please note that it must be consistent with the subject The name registered on the (identity) document remains consistent.

Individual: Name + ID number: Foreign applicants should fill in both their Chinese name and English name.

2. Fill in the "Applicant's Nationality/Region" column: Domestic applicants do not fill in this column; foreign and Hong Kong, Macao and Taiwan applicants should fill in their nationality or region correctly and correctly.

3. Fill in the "Applicant Address" column:

4. Fill in the "Agency Name" column: The agency name should be consistent with the agency stamp stamped on it.

5. Fill in the "Domestic Recipient of Foreign Applicant, Domestic Recipient's Address, and Postal Code" columns: "Domestic Recipient of Foreign Applicant", "Domestic Recipient's Address", and "Postal Code" columns are for foreign applicants to fill in. The foreign applicant shall designate a domestic receiver in the application form to receive the legal documents for the subsequent trademark business of the Trademark Office and the Trademark Review and Adjudication Board. The address of the domestic recipient should be filled in in detail with administrative divisions such as province, city, and county. Domestic applicants do not fill in this column.

6. "Trademark Application Statement" column: Application for registration of collective trademarks and certification trademarks. If applying for trademark registration with a three-dimensional mark, color combination, or sound mark, or if two or more applicants apply for registration of the same trademark at the same time, they should declare in this column. Applicants should make selections based on the application content and attach relevant documents.

7. "Priority Claim Statement" column: If the applicant requests priority according to Article 25 of the "Trademark Law", select "Priority based on the first application" and fill in the "Application /Exhibition Country/Region”, “Application/Exhibition Date”, and “Application Number” columns. The applicant should submit priority certification documents (including originals and Chinese translations) at the same time; if the priority certification documents cannot be submitted at the same time, the applicant should choose "priority certification documents later" and submit them within three months from the filing date. If a written statement is not made or priority certification documents are not submitted within the time limit, priority will be deemed not to have been claimed.

Priority certification document refers to a copy of the first trademark registration application document submitted by the applicant. This copy should be certified by the trademark authority that accepted the application and indicate the application date and application number.

When Taiwanese applicants apply for trademark registration and request Taiwan priority, they should use the "Trademark Registration Application Form (For Taiwan Applicants Only)".

8. "Stamp and signature": All stamps that need to be affixed to the application should be official seals; the official seals of the applicant and the agency should be stamped in the corresponding positions, and the official seals must be clear. Complete and must not cover the text entered in the application.

9. "Trademark pattern frame": The trademark pattern should be clear, complete and contrasting. If you apply with a color combination or specified color pattern, you should submit the colored pattern and a black and white ink draft; if you do not specify a color, you should submit a black and white pattern.

10. "Trademark Description" column: Applicants should fill it in according to the actual situation. If you apply for trademark registration with a three-dimensional mark or sound mark, you should explain how the trademark is used. If you apply for trademark registration based on a color combination, you must submit a text description, indicate the color code, and explain how to use the trademark. If the trademark is in a foreign language or contains a foreign language, the meaning shall be explained. When a natural person applies for registration of his or her own portrait as a trademark design, an explanation must be given. When an applicant applies for registration of another person's portrait as a trademark design, he or she shall explain the registration and attach a statement from the person who created the portrait. 11. "Category. Goods/Services" column: Trademark registration applicants can apply to register the same trademark for multiple categories of goods through one application. Applicants should fill in the category and name of goods/services according to the "Classification Table of Similar Goods and Services". Goods/service items should be filled in according to category, and the sequence number should be marked in front of each category of items. If the categories and goods/services cannot be filled in, they can be filled in on the attached page according to the format of this application. After filling in all categories and projects, the word "Deadline" should be marked.
