To apply for a patent contract, the following documents shall be submitted in duplicate:

(1) An application form for filing;

(2) A copy of the contract;

(3) A copy of the patent certificate or the notification of acceptance of the patent application;

(4) Identification certificates of the transferor and the transferee;

(5) Power of attorney;

(6) Other documents.
