Description:

1. The applicant refers to a natural person who has established individual industrial and commercial households. If the applicant entrusts an agent, it shall submit the power of attorney of the applicant and the identity certificate or qualification certificate of the agent (if the applicant is the operator himself, it is not necessary to submit the certificate of entrusted agent).

2. If the original and photocopy of the business license are not submitted, a sample with a loss statement must be submitted.

3. The documents and certificates submitted by the applicant shall be original. If the original cannot be submitted, the copy shall be marked "consistent with the original" and signed by the applicant.

4. The applicant shall fill in the form or sign it neatly with pen, brush or signature pen.
