Operators with valid food business licenses are involved in the following matters:
1, engaged in food business through the Internet, and filed with the original licensing authority within 30 days from the date of accessing the Internet;
2, the central kitchen food varieties or distribution stores change;
3, food sales chain headquarters, catering service chain headquarters and catering enterprises and other food business units subordinate food stores or contractors canteen changes;
4, food safety management personnel changes;
5, herbal tea shop operating varieties or herbal tea raw materials list changes;
6, the main facilities and equipment changes.
Materials required:
1, food business license filing application;
2. The original and duplicate of the food business license;
3, engaged in food business through the Internet, within 30 days from the date of its access to the network, its website, IP address, website selling food, web page or home page screenshot of online shop and other information should be filed with the original licensing authority;
4, the central kitchen distribution stores change, submitted to the central kitchen distribution stores for the record;
5, food sales chain headquarters, catering service chain headquarters and catering management enterprises and other food business management units subordinate food stores or contracted management unit canteen changes, need to submit the "food business management unit subordinate stores list change record form";
6, food safety management personnel changes, need to submit the "food safety management personnel change record";
7, herbal tea shop business varieties or herbal tea raw material list changes, need to submit the list of herbal tea raw materials change for the record;
8, the main facilities and equipment change, need to submit the main facilities and equipment change record;
9. If the applicant entrusts others to apply for a food business license, the agent shall submit a power of attorney.

Article 4 Food producers and operators shall be responsible for the food safety of their production and operation.
Food producers and business operators shall engage in production and business activities in accordance with laws, regulations and food safety standards, ensure food safety, be honest and self-disciplined, be responsible to the society and the public, accept social supervision and assume social responsibilities.
