4. If the name and address of the registrant are corrected after the change is completed, the original certificate of change shall be returned at the same time.

5. If the name and address of the transferee registrant are corrected, the original transfer certificate shall be returned at the same time.

6. If the trademark registration certificate is changed due to the correction of wrong information, the original trademark registration certificate shall be returned at the same time.

(2) Specific requirements

1. The applicant shall fill in the application documents truthfully as required, and shall not modify the format without authorization. The application should be printed out.

2. The name of the applicant and the seal (signature) affixed to the seal (signature) of the applicant shall be consistent with the name in the identity document. If the applicant is a natural person, the ID number shall be filled in at the back of the name.

3. The address of the applicant should be preceded by the name of the administrative division such as province, city and county. The applicant should fill in the address on the identity document. If the address in the identity document is not marked with administrative divisions such as province, city and county, the applicant shall add the name of the corresponding administrative division. 5. The application form shall fill in the application items for the correction of the trademark application number (registration number), and indicate the corrected items and contents in the column of the application form.

6. If the name/address of the trademark applicant is corrected, all categories of trademarks shall be filled in.

7. The corrected contents do not involve the substantive contents of the trademark application documents or registration documents. The items to be corrected shall indicate the items and contents of the application for correction.

8. If you need to reissue the certification documents, you should choose "Yes" and attach the wrong trademark registration certificate or the original documents such as the certificate of change, transfer and renewal.
