View>Toolbar>check the form, then select your radio button or check button, click it, draw a small box where you want to set it, and then right-click it. Change the content you want to set in it.

First select the entire worksheet, format the cells/unprotect, then select the cells that are prohibited from modification, format the cells/protect/check "Protect" " and "Hide", Tools/Protect/Protect Sheet, check the edit object when protecting.
