You can apply for a credit card of China Construction Bank in official website, choose the type of card you want to apply for, and then just write it as above. You can submit it after writing, but there seems to be a time limit (10 minutes or so). After the submission is successful, the bank will call you to check the information (the phone number is not necessarily 95533), and the check is basically passed. Then, if you have a savings card of CCB, log in to CCB's personal online bank, see the application number in the column of applying for a credit card (click it and you will see the words of application form submission, etc.), then send you a text message after waiting for 95533 (don't delete it), and go to any CCB for a credit card interview within 7 days after receiving the text message (just give you a form for your signature, and you need a text message and ID card sent by 95533). Then I'll wait for 95533 to send you a card (your credit card named xxxx has been mailed, and the number xxxxxxx is probably these, I forgot the details), and then your credit card will be available.
