Involuntary unemployed persons who participate in unemployment insurance in our district shall, within 60 days from the effective date of the ruling or judgment of termination or dissolution of labor relations or labor disputes, apply for unemployment insurance benefits from the social security institution of the town (street office) with the following information:

1. One original and one copy of my ID card;

2, the original work unit to terminate or terminate the labor relationship certificate;

3. Provide labor manuals for local household registration and employment certificates for migrant workers for household registration in other regions;

4, fill in the "unemployment insurance application form" or "unemployment insurance one-time living allowance application form".
