1) Trademark agency power of attorney signed or stamped by the trademark change applicant;

2) Trademark change application signed or stamped by the trademark change applicant;

3) Original or stamped copy of the change certificate issued by the industrial and commercial department (change of name of registrant), copy of business license (change of address);

4) Copy of trademark registration certificate or A copy of the acceptance notice.
