2. Specific instructions:

(1) Fill in the application form one by one, and it must be typed or printed. If the applicant is a natural person, he/she should fill in the ID number after his/her name, and if a foreign natural person fills in the passport number

(2) When going through the alteration formalities, each trademark (each registration number) should fill in an application form for alteration and affix the seal of the applicant, and if it is handled through a trademark agency, the agent's seal is also required.

(3) Other documents for handling the change formalities need to be signed or sealed by the applicant for confirmation.

(4) If the trademark applied for change is a trademark of * * *, the following documents shall also be submitted:

1. If there is a trademark representative in the application for change of * * *, an application for change of the name/address of the registrant shall be filled in, and a statement of consent of other * * * owners shall also be submitted.

2. If you apply to change the name of the trademark * * *, whether it is to change the name of all * * * owners or some * * * owners, or to change the name of the trademark representative, you should fill in the names of all * * * owners in the application form. The name and address of the representative should be written on the first page of the application, and the names of other * * * people should be written on the attached page of the application, and other * * * people do not need to fill in the address.

The application is charged by category, and the change fee for one category is 5 yuan RMB.

V. Precautions

1. If the name or address of a trademark registrant is changed, the trademark registrant shall change all its registered trademarks together.

2. After the change application is submitted, the Trademark Office will accept the change application that meets the acceptance conditions; Do not meet the acceptance conditions, will not be accepted.

3. If the change application needs to be corrected, the Trademark Office will issue a notice of correction/correction, informing the applicant to make correction/correction within a time limit. If the applicant fails to make corrections as required within the prescribed time limit, the Trademark Office has the right to regard the application for change as abandonment or disapproval.

4. After the application for change is approved, the Trademark Office will issue a certificate of change to the applicant.

6. After the trademark change application is approved, the change certificate will only be issued to the representative. If other people need it, they should apply for a replacement certificate.

7. If the applicant directly handles it, the Trademark Office will send the corresponding documents to the applicant by mail according to the address filled in the application; If it is handled through a trademark agency, it shall be served on the agency.

8. The category of the application shall be filled in according to the international classification approved by the Trademark Registration Certificate.

9. In case of changing the agent/domestic recipient who has not used the application for cancellation, objection, objection review or rejection of registration review for three consecutive years, the application for changing the agent/domestic recipient shall be submitted directly to the trial department of the procedure, and the agent/domestic recipient for changing, transferring, renewing or canceling the application cannot be changed.
