4. When filling in the "Agency Name" column, the agency name should be consistent with the agency stamp stamped on it.

5. Fill in the "Domestic Recipient of Foreign Applicant, Domestic Recipient's Address, and Postal Code" columns. columns are for foreign applicants to fill in. The foreign applicant shall designate a domestic receiver in the application form to receive the legal documents for the subsequent trademark business of the Trademark Office and the Trademark Review and Adjudication Board. The address of the domestic recipient should be filled in in detail with administrative divisions such as province, city, and county.

Domestic applicants do not fill in this column.

6. In the "Trademark Application Statement" column, if you apply for registration of a collective trademark or certification mark, or if you apply for trademark registration with a three-dimensional logo, color combination, or sound mark, two or more applicants must apply for registration at the same time. If they have the same trademark, they should be declared in this column. Applicants should make selections based on the application content and attach relevant documents.

7. In the "Declaration of Priority Request" column, if the applicant requests priority in accordance with Article 25 of the Trademark Law, select "Priority based on the first application" and fill in the "Application /Exhibition Country/Region”, “Application/Exhibition Date”, and “Application Number” columns. The applicant should submit priority certification documents (including originals and Chinese translations) at the same time; if the priority certification documents cannot be submitted at the same time, the applicant should choose "priority certification documents later" and submit them within three months from the filing date. If a written statement is not made or priority certification documents are not submitted within the time limit, priority will be deemed not to have been claimed.

The priority certification document refers to the copy of the first trademark registration application document submitted by the applicant. This copy should be certified by the trademark authority that accepted the application and indicate the application date and application number.

When Taiwanese applicants apply for trademark registration and request priority in Taiwan, they should use the "Trademark Registration Application Form (For Taiwan Applicants Only)".

8. "Stamp and signature", the stamps that need to be affixed to the application should be official seals; the official seals of the applicant and the agency should be stamped in the corresponding positions, and the official seals must be clear and Complete and must not cover the text entered in the application.

9. "Trademark pattern frame", the trademark pattern should be clear, complete and contrasting. If you apply with a color combination or specified color drawing, you should submit the colored drawing and a black and white ink draft; if you do not specify a color, you should submit a black and white drawing.

10. The applicant should fill in the "Trademark Description" column according to the actual situation. If applying for trademark registration with a three-dimensional mark or sound mark, the method of use of the trademark should be explained. If you apply for trademark registration based on a color combination, you must submit a text description, indicate the color code, and explain how to use the trademark. Other matters that the applicant thinks need to be explained can also be explained in this column.

11. "Category, Goods/Services" Column

Trademark registration applicants can apply to register the same trademark for multiple categories of goods through one application.

Legal basis: Article 25 of the "Trademark Law of the People's Republic of China" A trademark registration applicant shall, within six months from the date of first filing an application for trademark registration of his or her trademark in a foreign country, If a trademark registration application is filed for the same goods with the same trademark in China, the foreign country may enjoy priority in accordance with the agreement signed by the foreign country with China or the international treaty both parties are a party to, or in accordance with the principle of mutual recognition of priority.
