Materials required for reimbursement in different places:

1, general outpatient reimbursement needs to provide: household registration book or ID card, new rural cooperative medical insurance payment form;

2. Hospitalization reimbursement needs to provide: household registration book or ID card, new rural cooperative medical payment bill, hospitalization invoice, diagnosis certificate, expense list and hospitalization medical record;

3. Overall reimbursement for serious illness needs to provide: household registration book, payment documents for participating in the new rural cooperative medical system, hospitalization invoice, diagnosis certificate, expense list, hospitalization medical record, and first reimbursement voucher for combined treatment.

Processing flow:

1, I or my family contact the insured institution by telephone to apply for cross-provincial medical referral;

2. Choose a designated medical institution for cross-provincial medical treatment, otherwise it will not be reimbursed;

3, patients with ID card (or residence booklet), cooperative medical certificate, admission certificate, referral form (or referral message) for admission procedures;

4. When the patient is discharged from the hospital, he/she will settle at the service window of the new rural cooperative medical system. Pay attention to bring complete materials and pay for personal expenses.
