2. Grass-roots trade unions shall submit application materials to the trade unions of district and county bureaus.
3. The trade union of the district/county bureau will fill in and seal the examination and approval form for payment application (downloaded from the Internet) and submit the application materials for payment by the insured to the Municipal General Insurance Cooperative.
4. After receiving the application with complete materials and procedures provided by the insured, the Municipal General Insurance Cooperative shall, after investigation and verification, pay the serious illness insurance fund within 30 days (which may be extended under special circumstances) and incorporate the insurance fund into the service card of the trade union members of the insured.
