First of all, customers are advised to contact the sample acceptance department of our center for telephone consultation or appointment before sending samples.

2. First, fill in the inspection application form and sign a technical service contract with our center for the samples submitted by customers. In the technical service contract, the entrusting party shall provide the company name, taxpayer identification number, address, telephone number, bank of deposit, account number and other information, which shall be signed and confirmed by the customer representative and the entrusted agent of the center, and stamped with the official seals of both parties. The acceptance department of the center shall issue the inspection entrustment form according to the requirements of the entrusting party, and calculate the inspection and testing fees, which shall be signed by the managers of both parties for confirmation.

Third, the sample inspection requirements

(1) Health-related products

1. Product inspection needs to provide samples of commercial packaging, and new products can be temporarily packaged or similar to commercial packaging. The label on the package should at least include the sample name, specification, batch number, production unit and other information. The names of the samples submitted for inspection shall include brand names, common names and attribute names, and meet the requirements of naming standards for health-related products;

2. The customer shall provide the following products-related materials for inspection:

(2) enterprise standards and instructions for use;

(three) the inspection methods and basis of special inspection items, and the corresponding standards or reference materials;

(4) Other materials required for inspection.

3. According to the inspection dose requirements of various products, the customer sends enough inspection samples at one time, and the number of samples is charged at three times of the inspection dose, of which 1 is used for inspection, 1 is used for review inspection and 1 is used for arbitration inspection. At the same time, the samples submitted for inspection must be the same batch number/production date (3 batches of health food), and the samples submitted for inspection at different times with the same batch number/production date will not be accepted.

(2) Inspection requirements for drinking water

1, the inspection date of drinking water must be reserved with our center.

2. Customers collect water samples by themselves. The selection and cleaning of sampling containers, sampling methods, sampling volume, sample preservation and transportation refer to the relevant requirements of Collection and Preservation of Standard Test Methods for Drinking Water (GB/T 5750.2-2006). If the customer determines the microbial indicators, he can contact the center in advance to obtain the sampling container.

3. The customer should take samples according to the agreed date and send them to our center for testing as soon as possible.

(3) Relevant requirements for on-site inspection

1. For the detection of health hazard factors in public places, indoor air and workplaces, please contact our central monitoring office in advance, and call 029-82256602.

2. For the testing of clean operating room, auxiliary room and supply room in the hospital, air testing in purification workshop and clean laboratory testing, please contact the Institute of Disinfection and Biological Protection of our center in advance, 029-822 17444.

4. After the customer pays the inspection fee, the inspection and testing work can be officially started. If the payment is delayed, the estimated completion date will be postponed according to the actual payment date.

Five, after the completion of the inspection, the customer with valid documents in the business management office to receive the inspection report.

6. If the customer has any objection or needs to change the inspection report issued by the center, he shall submit a written application to the center within 30 days from the date of receiving the inspection report, and it will not be accepted after the deadline.
