To apply for a famous trademark in Jiangsu Province, the following materials shall be provided: (1) a list of application materials; (2) An application report on the recognition of famous trademarks in Jiangsu Province; (3) Application Form for Identification of Famous Trademarks in Jiangsu Province; (4) The business license of the enterprise as a legal person or a copy of the business license; (5) A copy of the trademark registration certificate applied for recognition, and a copy of the supporting documents such as transfer, alteration and renewal; (six) in the name of an interested party, the applicant must provide a copy of the trademark license contract signed with the trademark registrant and the power of attorney issued by the trademark registrant; (seven) the objectives, measures and effects of the enterprise to formulate and implement the trademark strategy and enhance its competitiveness and trademark value; (8) A legal statement that the main economic indicators of the goods (services) using the trademark in the last three years are consistent with those reported to the tax authorities; (nine) the tax payment certificate issued by the tax authorities, in line with the provisions of tax reduction and exemption, shall provide a certificate of tax reduction and exemption; (ten) a copy of the certification documents of the market reputation and award-winning situation of the enterprises and commodities (services) that use the trademark; (eleven) a copy of the well-known trademark, the provincial famous trademark identification document or certificate; (twelve) a copy of the overseas trademark registration certificate or application for registration; (thirteen) the damage and protection records of the exclusive right to use a trademark (such as complaints or appeals, administrative punishment decisions, court decisions and other supporting materials); (fourteen) a copy of the sales certificate of the sales area (prefecture-level city) where the goods (services) using the trademark are sold, and a copy of the customs declaration form for export goods; (fifteen) a copy of the advertising media and the fee voucher; (16) The implementation standard of the goods using the trademark, the three-year product quality inspection certificate issued by the quality inspection department (the certificate issued by the quarantine department for export products), the quality sampling record or the copy of the inspection exemption certificate and other related materials; (seventeen) the enterprise uses the goods (services) of the trademark to provide proof materials for repair, replacement, return and compensation, and to provide proof of three guarantees for the compulsory implementation of three guarantees; (eighteen) certification materials of enterprise trademark management organization, personnel and system.
